We are a licensed and Medicare certified Home Health Agency dedicated to serve the North Bay and East Bay’s diverse population. Our multidisciplinary team of health care professionals are well-trained to clinically address patient complex needs using a patient-centered approach. We assist patients in developing their treatment plan to intervene and prevent rehospitalization.

Clients who need transitional/intermittent care after recent hospitalization, clients with disability, chronic, terminal illnesses and those who need skilled services/assistance within the comfort of home..
Who is eligible for Home Health Services?
Those in need of therapy or skilled nursing assistance, those who are unable to leave home due to illness and have a physician’s order for visits per prescribed plan of care are eligible for our services.
How to Arrange For Care
Physicians or health care facilities can make referrals. The client or family member can also call to arrange for services. We will contact the client’s physician for approval before providing services.
Who pays for your Home Health Services?
Medicare and Medi-Cal, Military Insurance, Private Payment, and Private Insurance.
Areas of Coverage
We cover Solano, Napa, and Contra Costa counties.
